THE SOLUTION
The system that solved it.
ITS Lab's exam engine locks the browser to fullscreen, records every mouse click, keyboard event, and tab switch. If the student leaves fullscreen, a 10-second countdown begins before automatic submission. Camera feed and screen are both captured as screenshots every 5 seconds throughout the exam.
